Your care plan
Using information from the pre-admission assessment and any transfer letter from the hospital together with information from yourself and your relatives, the senior registered nurse responsible for your care will draw up an individual care plan reviewing your needs and prescribing the care you should be receiving.
You and your relatives will be involved in this process and we would ask you to sign the finished care plan which is then placed at the end of your bed for the Team Leaders and Health Care Assistants to read and then deliver your care.
Reviews of your care plan are carried out on a monthly basis. Reviews are more frequent if there are changes in your condition making it necessary to indicate a change to the level of the care you require.
